APPENDIX

 

This section includes items that may be of use to student organizations.  Following is a brief description of these items and how they can be used:

 

  1. Sample Agenda: An agenda is a document that serves as a ‘to do’ list of sorts.  It is useful when conducting meetings.  An agenda can help to set the flow of a meeting and inform participants of what to expect of discussion and when it is appropriate to discuss a particular item.
  2. Sample Goal Statement: A goal statement is a statement that defines in detail what goals you have set for your organization.  This can be done as a group activity at the beginning of the semester to get you started.  Of course, you may have more than one goal.  First, you should state your goal.  Next, you will come up with a specific objective that will help you meet your goal (there may be several).  Then, you will set an action plan as to how to carry out that objective (there may be several).  Upon completion of this activity, your organization will have a clearly defined set of goals that will guide your programming efforts and objectives throughout the year.
  3. Sample Memo: A memorandum (memo) is a written record or communication.  These are useful when communicating information to your organization or to other parties.  They may be less detailed than formal letters. 
  4. Sample Minutes: Minutes are an official record of the proceedings of a meeting.  They are usually kept by the recording secretary of an organization.  Minutes are useful so that the organization can look back to review what was discussed at a previous meeting.  It helps to hold everyone accountable.
  5. Sample Program Budget: A program budget is a list of items detailing income and expenses.  It assists an organization in managing its funds as well as projecting what kind of funding will be necessary for the upcoming year.
  6. Blue Requisition Form: This form is used for transfers or to ensure payment for contracts.
  7. Deposit Form: This form is used when an organization is depositing funds into its university account.
  8. Direct Pay Form: This form is used when your organization has received and invoice and it is necessary to pay a bill or when an individual or organization is seeking a reimbursement.
  9. Organization Registration Form: This form must be filled out and submitted in order to begin the registration process.
  10. Independent Contractor Agreement: This contract is used when a vendor/performer does not provide his/her own contract.
  11. Vendor Application: A vendor application is used to collect information from a vendor/performer and is submitted with a contract.
